I ordered an assembled Mendel reprap in mid December 2010 from botmill.com, using these links. Their contact info is support@botmill.com / 561-549-1669.
It arrived on campus late Feb 2011 ... in my office March 2. (Ah yes: the speedy modern age.)
The Mendel itself as I ordered it doesn't seem to be on botmill's website anymore. They're now listing an "Axis (unassembled)" and a "Glider (assembled)". Both seem to be close to the Mendel; the differences aren't clear.

In particular, I *think* the instructions for attaching the extruder are somewhere on this page:
Images of the unpacked gear are attached.
Why they chose to send philips/flat bits and a driver when all the bolts are hex is, I confess, a bit beyond me.
